Why This Happens
Your body is remarkably adaptive — but it’s also ruthlessly efficient. When you’re in an extended calorie deficit, it looks for the most energy-efficient way to downsize. Muscle tissue is metabolically expensive to maintain: it burns calories just by existing. Fat tissue is cheap to keep around. So without a clear signal to hold onto muscle, your body will happily reduce it alongside fat.
That signal? Resistance training. Specifically, progressive overload — consistently challenging your muscles with enough load that your body has a compelling reason to maintain them. Without that signal, your body has no reason to spend the energy keeping muscle you’re not using.

What Muscle Loss Actually Feels Like
Here’s why this matters beyond the abstract science: muscle loss has real, felt consequences that many GLP-1 users attribute to the medication itself or to getting older — when in fact, they’re symptoms of a body that’s losing its structural foundation.
- Fatigue and low energy — muscle is your body’s primary engine for movement and energy production. Less muscle = less vitality.
- Feeling “soft” despite losing weight — this is the phenomenon sometimes called “skinny fat.” The scale is lower, but the body composition hasn’t improved.
- Weight regain after stopping medication — a slower metabolism from muscle loss makes it dramatically harder to maintain weight without the medication’s appetite suppression.
- Joint pain and instability — muscle supports and stabilizes joints. Less muscle means more wear, more pain, and higher injury risk.
